Pisces helped OCS IT sharpen its niche, remove underperforming overhead, productize Joe Goldstein’s expertise, and turn an industry conference into a repeatable revenue channel.
OCS IT entered the engagement at approximately $1.65M in annual revenue and 22% NOI. It had developed unusual expertise serving ENT and adjacent specialty medical practices.
The problem was not that the business was broken. Joe remained the center of nearly every growth initiative. Sales depended heavily on his relationships, memory, conference attendance, and personal follow-up. Some employees, vendors, and marketing programs consumed resources without returning enough value.
The business needed sharper decisions about where to focus, what to stop funding, and how to turn its niche expertise into a stronger revenue advantage.
Deeper niche strategy. ENT and adjacent outpatient medical specialties became the operating lens for positioning, speaking, content, qualification, service development, partnerships, and event strategy.
Stronger cost discipline. Pisces coached Joe through decisions about underperforming employees, vendors, programs, and priorities so revenue gains were not absorbed by avoidable overhead.
Productized expertise. AI and healthcare knowledge became a revenue pathway: strategy session, readiness scan, workflow workshop, implementation guidance, and ongoing advisory.
A deliberate event strategy. ASCENT stopped being an annual networking event and became a coordinated system connecting authority, private conversations, offers, outreach, qualification, and follow-up.
The 2025 strategy produced qualified meetings, more than 40 decision-maker RSVPs, a 17% email reply rate, and stronger speaking visibility. More importantly, it produced revenue.
ENT Specialists of Wisconsin signed with OCS and began onboarding in 2026. Joe estimated the account at $110K–$150K in recurring annual revenue, with another $50K–$100K in anticipated project work. He described the win as roughly five times the new revenue OCS had generated during the previous two-and-a-half years.
Two additional ASCENT-sourced practices remained interested but were delayed by incumbent contracts. OCS was no longer one interchangeable MSP among hundreds. It was the provider that understood the technology, workflows, vendors, risks, and operational realities of an ENT practice.
At onboarding, OCS produced approximately $363K in annual NOI. At $2M in revenue and nearly 30% NOI, annual operating income is now close to $600K.
That represents approximately $350K in additional annual revenue, roughly $225K more annual NOI, and nearly eight points of margin improvement. OCS documented $38,300 invested with Pisces; the increase in annual NOI run rate is nearly six times that investment.
The comparison excludes potential project revenue from the new ENT client and the value of additional opportunities still in motion.
For ASCENT 2026, Pisces refined the strategy around a stronger vendor-neutral AI advisory offer for specialty medical practices.
Before the event, the first deployment had already produced 189 delivered emails, nine replies, two booked meetings, and zero unsubscribes. The meaningful result is not the reply rate by itself. OCS can now create qualified conversations around a broader service portfolio before the founder enters the room.
The event motion is no longer dependent on improvised outreach. It is becoming a repeatable revenue channel.
This case shows: An established founder-led MSP can improve revenue, margin, market authority, and pipeline when niche strategy, cost discipline, offer architecture, event strategy, and follow-up are treated as one system.
It does not show: That Apollo or any single campaign created the outcome, that every MSP will produce the same result, or that strategy removes the need for disciplined execution.
Results reflect the documented case-study period and are not guarantees of future performance.
Revenue Triage is a focused diagnostic for founder-led MSPs that need an evidence-based answer before committing to another tactic, hire, platform, or agency.
